Dr. Erik Albert Chamberjian

I received chiropractic services from Dr. Chamberjian in 2018. At that time I was informed by his office that I would be responsible for a co-pay of $30.00 a visit as they were an in-network provider. I paid that co-pay each visit. I later received a bill from him. After many months of investigation I was informed by my insurance company that Dr. Chamberjian submitted billing codes they did not cover, and I would have to address this bill with him. I was then informed by Dr. Chamberjian’s office that he administered additional procedures because they were the only method of treatment for my shoulder pain. Because he refused to speak with me in person, I wrote him a letter pointing out that it is my right as a patient and his obligation as a provider, to notify patients of procedures that are not a covered benefit, so that they have an opportunity to decline these services. I asked that he not bill me for these additional services or at least split the cost with me. Dr. Chamberjian
