Response to Ms. Sandra Miselem: 1. This guy shouldn't be allowed to practice as a geneticist. He's a general family doctor masquerading as an expert in a field he doesn't understand. My credentials as a Board Certified Medical Biochemical Geneticist and completion of Ivy League clinical genetics training are a matter of public record. I also was the recipient of a 2021 Excellence in Patient Experience award based on anonymous patient satisfaction surveys, scoring in the top 10% of physicians nationally on objective measures of me by my patients. 2. A critically ill friend of mine went to him for answers after *months* of waiting for an appointment. He dismissed her outright and refused to run a single test, didn't even try to see if insurance would approve them. Ms. Miselem is not my patient, and I would not comment on whether her friend is my patient or not. It is important to realize, however, that insurance companies have super-strict rules about coverage of diagnostic testing. Physicians generally know such rules by heart. It is unfortunately necessary in some cases for the patient to pay out-of-pocket for a non-covered test. Physicians really are not to blame for that. In addition, though patients have an almost absolute right to refuse diagnostic tests or treatments, they do not have an affirmative right to require a physician to order a particular test. If the physician declines, the best next step is to seek a second opinion from another physician. 3. Basically kicked her out of his practice. I have never discharged a patient from my practice for any reason. Any patient who ever needs help will receive it. In addition, there are many options for receiving an informal second opinion from the colleagues of a physician, if it would be a hardship for a patient to seek a formal second opinion. Such options remain open to any patient, at any time, and I would facilitate such consultations until the patient expresses full satisfaction.. 4. She is dying and this monster didn't care to look her in the eye and listen for 30 seconds to help her figure out why she's dying. This man deserves to lose his practice for the cruelty he inflicted on my friend. I empathize with the anguish that can accompany illness. I believe that physicians must absorb any anger or even unfair accusations that arise in the context of medical care. The ethical approach is to allow expression and ventilation of negative emotions, understand the problems that underlie them, and to seek solutions. Patients and their families & friends must know that it’s safe to express such emotions and that the physician would never hold it against them in any way. Being in the role of a patient can be terrifyingly disempowering and physicians must be mindful of that. I remain committed to help anyone who asks for help. Please remember, if ever irreconcilable differences arise between a patient and a physician, that seeking a second opinion is the best next step. It really works. I found Dr. Morgan to be incredibly kind, full of knowledge, patient, and concerned. After years of suffering from severe occipital headaches and debilitating neurological deficits, Dr. Morgan is the first Dr. in Tennessee I have ever heard to diagnose and acknowledge craniocervical instability-- something I'd suspected I'd had for the last 5 years. I didn't even mention it in my appointment with him. He was the first to say it and it was absolutely shocking and such a relief to be validated and understood- finally!I l wish the rest of Vandy doctors would educate themselves about hypermobility disordes and craniocervical instability/atlantoaxial instability. This state needs a good clinic an a place to go for care for patients like me whom suffer POTs and hypermobility conditions as well as the comorbities that go along with them. New York, Colorado and MUSC all have specialty clinics for people with these conditions are are ran by some of the top neurosurgeons in the world.
