Sad thing is that Dr Castellano may not know about this because his staff may have never given him the paperwork. My Mom had a rare epileptic seizure 6 months ago. I did all the paperwork properly and on time. It's day 4 and we are still waiting for the paperwork to be signed by the Dr to get her license back. We sent requests via the UPMS app and it said that it would take 2-3 days. We left messages on the answering service and it said that it would take 3 days to get back to a message. I did all that I could I now am desperate for help in getting this signed.

He switched medications on my wife without discussing it with us. She is having adverse reactions from it. Horrible doctor that showed no empathy. When he made her cry because of the unsympathetic way he talked about her condition, he didn't even have the decency to hand her a tissue as to dab her tears even though the box was right next to him. I had to tell him to hand her the box and he acted surprised as if human emotions perplexed him. Horrible doctor to deal with epilepsy and seizure disorders about.
