Never got the chance to have an appointment. I was previously a patient there but got new insurance and wanted to come back bc they’re close to home. I called to make an appointment with Dr. Usha for Feb. 1st. Showed up and was informed that my appointment was canceled because she isn’t taking new patients. I was never notified of this. I was disappointed bc the last visit I had there with my old insurance I was told that I needed blood work and wanted to take care of that now that I have new insurance. I made another appointment while I was there and was told it would be for Feb 3rd ( that coming Friday) and was given a confirmation print out to take home. Got home and looked at the confirmation to see that the receptionist made the appointment for the following Friday (Feb. 10 th). I give up. Currently looking for a new doctors office
I’ve been seeing Dr. Cook for over a decade. He is consistently well mannered and concerned for not only immediate conditions, but long term health. St. Agnes / Ascension just recently upgraded their facility on Route 40 - it’s a very modern and clean facility with mostly excellent support staff. I love the option for a virtual visit, if appropriate.
Dr Williams cook and his staff are the absolute best if you can get in with them never leave wait time is always 15 min or less in and out
Dr. Cook has been my primary care provider for a number of years. He is very knowledgeable and treats me with respect and professionalism all times.

Both I and my wife have been seeing this doctor from his original practice and now with st agnes. Anytime we needed referal or any other issues he would get back with us. Doc and his colleagues always treat us with care and professional.
I have been seeing Dr Cook for years. Always, he has responded to every concern & question with sensitivity & kindness. He listens carefully & responds to all of my concers.I feel very well cared for.This is an invaluable treasure to an aging body

Takes his time and asks questions, don’t feel rushed or that he doesn’t care.
Two visits were scheduled; Sep 29 and then Sep 28. BOTH were cancelled. Another appointment was made for Oct. Dr. Cook is an excellent doctor and I want to keep him as long as he wants to keep me.

I enjoyed my visit with Dr. Cook althoigh he wasnt my regular primary physician. I was very comfortable with him and information he gave me.He was very pleasantand didnt mind answering questions.

The visit was fine, The exceptionally long wait was unfortunate. Overbooking is simply counterproductive to quality care.

I have been a patient of Dr. Cook's now for years. I would highly recommend him to anyone looking for a new doctor. He takes the time and listens to you. Never makes you feel rushed. I feel like he genuinely cares and you aren't just another patient to him. I don't know what I will do when he retires!!!!
Dr. Cook has been my primary care physician for almost 10 years now. He's incredibly knowledgeable about a wide range of medical specialties. He's very patient and always willing to explain everything in detail. His office is very technologically advanced, and as a result waiting times seems almost non-existent. The practice includes many other providers who seem willing to accommodate when Dr. Cook is not available. With that said, I am almost always able to get a same-day appointment.
St. Agnes Practice including Dr. Cook and Dr. Levikas have been my family doctors for over 15 years and I would highly recommend them. They are attentive, responsive, competent, and very personable and take the time to educate their patients on medical issues.
