Dr Achebe signed my mother's Death Certificate. He never treated my mother. Nor did he take the time to read her medical history. If he had, he would have known that my mother had died from more than just Respiratory Failure. She died from Covid. This was not on my mothers Death Certificate at all. Not even under, "Direct Contributing Factors Leading to Death." Left blank. Among other items that were left blank. I have tried to contact him thru Acuity. Its been about a week now, still nothing. Under FEMA, I can apply for funeral cost reimbersement. It needs to state she had COVID however on her Death Certificate. We are not allowed to visit with my mother for a half hour before she is cremated because she had Covid. Now, I find out it wasn't even listed on her Death Certificate. Not only that, but he has not responded to any contact made to him. I am waiting for him to correct my mother's Death Certificate before it can be certified. I guess he thinks it already is certified. Fortunately, I chose a BBB Crematory, which allows you to check for errors BEFORE it is certified. Dr. Acebe, PLEASE FIX MY MOTHER'S DEATH CERTIFICATE SO IT IS ACCURATE! Oh, and I DO NOT recommend Acuity. My mother went downhill fast when she was transferred there. Do not send loved ones there.

Great doctor. I have been visiting Dr. Achebe for several years.

Dr. Achebe is one of the best medical doctors I know. He listens, advises and follows up. He is very polite and empathetic. Seems to be quite bright too.

This place is dirty and smells like cigarettes. Ive been in many times for all different reasons and with barely getting a checkup or even my temp taken i was prescribed ibuprofen, antibiotics and heavy duty cough meds. Didnt want to give me a lymes test and i ended up in the ER a week later. He had the audacity to question the authenticity of the doc in the ER who treated me for lymes even after showing him my blood work. The staff here is friendly but the equipment is old& slightly rusty.

Love Dr. Achebe, he's the best PCP I've had in years, very thorough, plan on being a patient for years to come, keep up the good work Dr. Achebe!
