I was nervous to find another PCP after my previous doctor moved but thankfully I decided to book an appointment with Dr. Kuzdak! He was very friendly and listened to all of my needs while making me feel very comfortable. The office was nice and the staff was extremely helpful and nice. I’ve already referred my sister.

Not thorough. Never explained that updates would be posted on patient portal. Appointments seemed rushed and always seems to be researching information on the internet while we are talking.

The doctors at Farmington Village Family Practice are top notch. I have been coming here from over 20 years.

Excellent Doctor who still takes time to listen in this day of assembly line physicians.

Dr. Kuzdak and staff are always professional, caring in there services, and helpful. They take the time to listen and that is important today.

Started going to Dr.Kuzdak after my doctor of 20+ years retired on the recommendation of his nurse and I am very pleased. Dr.Kuzdak honestly cares for his patients and it shows.
