Mr. David B Jack

Dr Jack was very interested in my concerns about my health issues. He listened And responded to all my questions. Recommended lab work and expedited them so results were back the next day.

Annual physical and checkup

I have been going to Dr. Jack for several years. I have found him to be an adequate doctor for the most part. At times I have found him to be brisk and that my concerns were rather trivial. However, I have continued to go to him. My last exchange changed my opinion of him, sadly. I had a question about why I was alway billed for my physicals. I had a most unfortunate exchange of correspondence with him. He was very rude, and tivializing things that were supposedly brought up in the exam, that may have been discussed several years ago. I was very hurt and disappointed by this treatment. I feel like it was very unprofessional and disrespectful of me as a patient and as a person.

I have been seeing Dr. Jack for the last 15-20 years. He is always professional and personable. He has extensive medical knowledge and he always discusses the issues with thoroughness. I highly recommend him to anyone.

I have relied on and had Dr. Jack for many years. He has always been very direct and complete in his evaluation of health problems I've had. I actually attribute my life to him for detecting my prostate cancer 12 years ago. His professionalism is much apprecitated.

Dr. Jack always shows a caring heart. He listens intently to what we say then makes sure he understands it exactly. He is upbeat and happy and always smiling. He offers advice on exercises to relieve sore muscles and gives a handout for added information. My husband and I are very happy that we have chosen to use Dr. David Jack as our primary care physician.

Dr. Jack is a great doctor. He has been taking care of me for years. He is always willing to answer all my questions and address my concerns. He never rushes me through an appointment. He is very personable and easy to talk to. I trut him with all my health issues.

Dr. Jack's office is very friendly and helpful...I have always been able to get in for a same day appointment when needed and the wait for a later appointment is never long. They are very competent and have taken care of us well for many years.

Dr. Jack is very knowledgeable and easy to communicate with. He takes time to listen and explains whatever might be going on, in a very clear and precise manor, and then provides a plan of action.

Dr. Jack is a very caring man as well as being very intelligent. He is always willing to listen to you and looks you right in the eye when you are talking to him.I never feel like I am being rushed in or out. I WOULD DEFINETLY give him a 10 out of 10 star rating! He does a great job of explaining medical conditions. never too long of wait time. I have total trust in his decisions. Staff are friendly and helpful. Office is clean and comfortable.

Dr Jack has been my personal physician over the last twenty years or so. His medical expertise steadily grows.He is very personable. He is very effective in coordination of Tricare prescription needs. He provides outstanding referrals to specialty physicians, medical facilities, and Labs. I would highly recommend him as a personal physician!

Dr. Jack has been my primary care physician for over 20 years. I have told him he doesn't get to retire till after I've died (I'm 79)! I've seen other providers at the office when he wasn't available and I feel comfortable with any of them. I can't recommend their offices too strongly.

Dr Jack has been my primary care Dr for at least 10 yrs. He's gotten me through quite a bit these last 10 yrs too. When I've needed a specialist, his recommendations have always been spot on. When I see him for physicals or for other medical needs, he always goes out of his way to make sure my needs are met and my questions are answered regardless of how long it takes. Dr Jack is a solid 10. MSH

Dr Jack is awesome! One of the things I appreciate about him is how comfortable I feel in discussing my medical needs. I know that sounds like the average thing to say BUT it is so true with him! He truly cares. Sometimes I feel I am being a big baby but he makes sure to validate my concerns. He is kind.

I would not hesitate to recommend Dr. Jack to anyone. He explains medical conditions and concerns on a level of understanding between Dr. and patient. He is open to dialogue with his patient to promote solutions with health concerns.

Dr David Jack has been my personal physician for over 15 years and I have the utmost respect for he and his caring staff. Dr Jack and his medical staff have always treated me and my family with courtesy and superior customer service. They behave in a professional manner. I would highly recommend new patients to seriously consider having the Lone Peak Family Medicine take care of your medical needs. If they can't handle it then they refer you to a medical specialist who can.

Dr, Jack is an excellent physician. He never seems to hurry with and takes the time to answer my questions.

Highly recommended. Unfortunately he's not accepting new patients bit other providers in the clinic are.

What I appreciate most about Dr. David B. Jack is the easy way he has of talking and discussing my health cares, and giving me his opinions on what he thinks will benefit me. He always keeps up on the latest studies of medicine so I feel like I get cutting edge advice each time I see him. If I need a quick appointment I can see him or one of the other doctors in his office within 24 hours. He has been my primary care physician for 16 years and I would never think of going anywhere else.

He is very considerate, thorough, and friendly. He explains everything so that it is understandable.

Dr. Jack really cares about his patients. We love his positive, upbeat, knowledgeable bedside manner. My parents, sister, diabetic daughter, husband and myself think he is the best doctor any of us have ever had. A one in a million!
